- Insurance cardFront and back. Skip this if you're paying cash.
- List of current medicationsNames and doses.

- Our team will reach out shortly by email or phone to confirm your appointment time. Submissions sent outside of business hours will be reviewed the next business day.
- We accept Blue Cross Blue Shield, Aetna, Cigna, and Medicare. Cash-pay is also welcome — just let us know during intake.
- Yes. Telehealth visits are available for new and established patients in the states we're licensed in. For appointments that need a hands-on physical exam or in-office testing, your provider will recommend an in-person visit at one of our three South Florida offices.
- Yes. All information you submit is encrypted in transit and stored in our HIPAA-compliant system. Only your care team has access.
